Simple Format Document 1# - General Enduring Power of Attorney

This Simple Power of Attorney Format provides for the appointment of a spouse or partner or someone nominated by you to act as your attorney. You will also be able to nominate one or two alternate attornies who will act as your attorney if your first attorney is unable or unwilling to act.

If you appoint two alternate attornies you will need to indicate whether they must act jointly (together) or jointly and severally (together or apart).
